Consent affidavit by parents for passport of minor children applied in India

  • Passport offices in India demand an affidavit from parents if both of the parents or one of the parents has not signed Annexure H, which is required to be submitted along with the passport application of a minor child. If the parents are abroad, such an affidavit has to be attested by the Indian Mission/Post concerned.
  • Parents desirous of obtaining such an affidavit may download the affidavit from here [pdf file]. The affidavit will be on plain paper.

    Applicants for the affidavit should submit the following:

    • self attested copy of passport of both the parents
    • original or self attested copy of birth certificate of the child issued by local authorities
    • original or self attested copy  of marriage certificate
    • One self-attested photocopy of the Chinese residence/work visa.

  • One self-attested photocopy of the first and last two pages of the Indian passport of the applicant. [please bring along original passport, residence/work permit for verification and return]
  • One self-attested photocopy of the Chinese residence/work visa.
  • Fee RMB 184 in cash. 

Submission

The completed application along with enclosures should be submitted at the counter of the Consulate General of India, Shanghai between 0930 and 1200 hours on any working day


Delivery

You can collect the attested affidavit from the Consulate between 1600 and 1700 hours on the same day of submission.
